Application to Request for ACPE-Accredited CE Program 

To get ACPE-accredited Continuing Pharmacy Education accreditation for your educational program for pharmacy professionals, please complete these two online forms (#1 and #2)and send us the documents listed below (#3-#5) to HUCOPCPEoffice@Howard.edu or anthea.francis@howard.edu to start the application process  

 

1. APPLICATION TO REQUEST ACPE ACCREDITED CE PROGRAM - 38 Questions 

2. BIO & CONFLICT OF INTEREST FORM - 19 Questions 

3. E-mail the speaker’s CV/RESUME 

4. E-mail the speaker’s POWERPOINT PRESENTATION 

5. E-mail the PROGRAM FLYER 

 

GUIDELINE HOW TO PREPARE A SPEAKERS OR POWER POINT SLIDES SHOULD FOLLOW THIS GUIDELINE: 

· Maximum of 45 slides per CE credit 

· The FIRST SLIDE should include Title of Presentation, and the Name, Title, and Academic/Professional Credentials of the Speaker (s) 

· The SECOND SLIDE should include the disclosure statement. If the speaker (s) do no have any disclosure, state “Nothing to Disclose” 

· The LAST SLIDE should include the “ACCESS CODE” and instruction on how to claim CE Credit. This will be provided by the CPE Office afterward.  

· All the references should be listed on the slide next to the last
